PA Department of Human Services (DHS) Family and Youth Survey

The Pennsylvania Department of Human Services (DHS) is seeking the input of youth, young adults, and family members who have lived experience with receiving services and supports from many different systems, such as mental health, intellectual or developmental disabilities, child and family welfare services.  This information is needed to better understand their needs and to improve the systems of care supporting them.  This confidential survey will take about 10 minutes to complete.  Please use the link below to access the survey between 4/25 and 5/12. You may receive this invitation to the survey from more than one organization, if that happens please only complete it once.
